The athletic departments at Carroll County Middle and High schools have implemented a new system to track student physicals and medical histories related to their sport.
Final Forms is the name of the new program which will replace Event Link only for physicals. Scheduled events will still be maintained through Event Link.
Students should give a paper copy of their physicals to their coach, who will upload it to Final Forms.
“It is very important that all student athletes sign up for Final Forms because this is where their physicals, contact information and medical history will be stored for our coaches and athletic trainer in case of an emergency,” said Athletic Director Scott Anderson.
Once the information is uploaded to Final Forms, it will remain through the student’s tenure at Carroll County Schools. Multiple sports can also be selected within the account so students and parents only have to enter the information one time.
Parents and students should both create accounts in Final Forms. It is important that all pieces of the form are completed or students will not be eligible to play.

Los departamentos de atletismo de las escuelas secundarias y preparatorias del condado de Carroll (CCMS y CCHS) han implementado un nuevo sistema para realizar un seguimiento de los exámenes físicos y los historiales médicos de los estudiantes relacionados con su deporte.
Final Forms es el nombre del nuevo programa que reemplazará a Event Link solo para exámenes físicos. Los eventos programados se mantendrán a través de Event Link.
Los estudiantes deben entregar una copia impresa de sus exámenes físicos a su entrenador, quien la cargará en Final Forms.
“Es muy importante que todos los estudiantes atletas se registren en Final Forms porque aquí es donde se almacenarán sus exámenes físicos, información de contacto e historial médico para nuestros entrenadores y preparador físico en caso de una emergencia”, dijo el director deportivo Scott Anderson.
Una vez que la información se carga en Final Forms, permanecerá durante el período del estudiante en las escuelas del condado de Carroll. También se pueden seleccionar varios deportes dentro de la cuenta, por lo que los estudiantes y los padres solo tienen que ingresar la información una vez.
